(a) The board shall keep a record of its proceedings. (b) The board may keep a record of any information the board wants to record. The board shall keep a record of: (1) the name, age, and present legal and mailing address of each applicant for examination; (2) the name and location of the school of optometry from which the applicant holds credentials; (3) the time devoted by the applicant to the study and practice of optometry; and (4) whether the board rejects or licenses the applicant. (c) A certified copy of the record described by Subsection (b), marked with the hand and seal of the executive director, is: (1) admissible evidence in all courts; and (2) prima facie evidence of all matters contained in the record. (d) The executive director shall number and record each license or renewal certificate issued by the board.
